To search for a facility enter one or more search criteria:

Click [Search] to begin the search for the facility. To select the facility from the list of search results, highlight the facility by clicking on it. To complete the selection click [Select Facilities].

Two other options on in this screen are the ability to create a new facility or edit the information of an existing facility.
