Patient Chart

The 'Patient Chart' is the repository of all the data related to a single patient. This can be compared to a folder where all patient documents are filed under various sections divided by tabs. In a similar way, under the patient chart we have several tabs for organizing different types of records. The tabs are defined by each practice/provider based on their needs.

HPN chart allows patients to access their data stored in patient chart by creating them a user account.
